New Patient Information
Your First Visit
We’ve worked hard to create the ideal experience for your initial appointment. After being warmly greeted and filling out any remaining paperwork, our excellent office manager will bring you back to take X-rays and discuss your chief complaint and general goals for your dental care with us. Then, Dr. Patel will step in and review your X-rays, as well as perform an exam. He will explain his findings in detail with you, showing you the actual areas of damage or decay, before answering any questions you may have. After deciding on feasible treatments and scheduling any needed follow-up appointments, our office manager will assist you with payment – and that’s it!
New Patient Forms
Having to wait until you arrive at the dentist to fill out your paperwork can be a big hassle. Instead, why not complete them in the comfort of your own home beforehand? By following the links below, patients can download the forms they’ll need, print them out, and write in the pertinent information whenever they have the time. Make sure to bring them with you to your first appointment, and you’ll be brought back to meet Dr. Patel that much sooner.
